The treatment coordinator will assist you in answering all of your questions in scheduling appointments, coordinating with the hospital, dental insurance billing and provide you with pre and post care instructions.
General anesthesia is covered under your medical insurance. Once we submit the medical insurance information to the hospital, the hospital will contact you regarding required pre-authorizations to cover percentage of general anesthesia and hospital facility charges. Please, allow the hospital at least 2-3 weeks to obtain the pre-authorization from the medical insurance company. The dental office does not manage or acquires medical authorizations for the hospital, therefore please call hospital billing at (630) 856 - 8400 if you have any questions.
A few days before the surgery you will receive a telephone call from the hospital to arrange for a Pre-admission testing appointment.
Prior, to the Pre-admission testing appointment, a HISTORY and PHYSICAL EXAMINATION and certain Laboratory tests will be needed to be performed by Child's Pediatrician.
***Please, bring completed History and Physical forms and additional laboratory tests requested to the Pre-admission testing appointment. Also, please fax the completed forms to the dental office at (708) 387-8206 or email it to Info@LifeKids.com for our records.***
During your advance visit to the admitting department the Nurse will pre-register your child, review History and Physical, discuss financial arrangements and give you surgery instructions. Verify your time for surgery at Hinsdale Hospital. Answer any questions you may have before the surgery.
NO FOOD OR DRINK SHOULD BE GIVEN TO THE PATIENT AFTER 12 MIDNIGHT BEFORE THE SURGERY. FAILURE TO COMPLY WITH THE GUIDELINES WILL RESULT IN CANCELLATION OF THE PROCEDURE.

Our office requires 100% deposit of the estimated dental treatment one week prior to the scheduled surgery date. The remaining balance is due on the day of the surgery. Failure to comply will result in cancellation of the procedure.
